What do I need to bring?

Certified birth certificates, government issued photo I.D., Security numbers, proof of residency, and if previously married, a filed copy of the final judgment of divorce, legal annulment or certified death certificate must be shown at the time of application.

How To Apply:

Each applicants must present a certified copy of their Birth Certificate, government issued photo I.D. and Social Security number. If either applicant was previously married, copies of judgment of divorce, legal annulment papers, or death certificates from previous marriage must be presented.

When does It Expire:

The license is valid for 30 days after date of issuance

Name Change:
Filing for a marriage license in the state of Wisconsin does not cause your or your spouse’s last name to automatically change. Married name change services are available to guide you through the married name change process if you opt to change your name.
