What do I need to bring?
Both applicants must provide a certified birth certificate, a valid picture identification and Social Security number. If you have been married before, you will need to to show a certified copy of the final divorce decree, with court raised seal, of your divorce or death certificate of deceased spouse.
How To Apply:
Both applicants must provide a birth certificate, a driver’s license or State identification card and Social Security numbers. If either applicant was previously married, divorce judgment including the date of the divorce and also the Judge’s signature or death spouse death certificate must be present at time of application.
When does It Expire:
The license is valid for 30 days after date of issuance and the marriage must be performed within that period.
Name Change:
Filing for a marriage license in the state of Wisconsin does not cause your or your spouse’s last name to automatically change. Married name change services are available to guide you through the married name change process if you opt to change your name.
