$120 Cash Only. No checks, no credit and no debit cards are accepted. Payment is due upon application and is non-refundable.
What do I need to bring?
Certified birth certificate, government issued photo ID, government issued document to show proof of residency for the past 30 days and Social Security numbers. f previously married, a divorce decree or death certificate of spouse is required.
How To Apply:
Both applicants are required to present certified birth certificates, Social Security numbers, government document showing residency for the past 30 days, and provide the date and place of marriage ceremony and full name and title of the officiant. If either applicant was previously married, a divorce decree and death certificate of deceased spouse.
When does It Expire:
You must then use your license within the next 30 days.
Name Change:
Filing for a marriage license in the state of Wisconsin does not cause your or your spouse’s last name to automatically change. Married name change services are available to guide you through the married name change process if you opt to change your name.
