What do I need to bring?
Proof of identification and residency (driver’s license and recent mail if address is incorrect on DL). State Certified birth certificates that must contain the official seal and signature of the County Register of Deeds or Bureau of Vital Statistics and Social Security numbers. If either applicant was previously married, spouse death certificate or divorce decree or annulment.
How To Apply:
Both parties must make application at the office of the County Clerk in the County of residence. Applicants must provide Social Security numbers, State certified birth certificate, and government document showing permanent residency.
When does It Expire:
The license is good for 30 days from issuance date.
