What do I need to bring?
Both applicants must provide a birth certificate, a government issued photo ID including permanent address and Social Security numbers. If either applicant was previously married, a certified copy of the divorce decree or death certificate.
How To Apply:
Both parties are required to present a certified copy of their birth certificate which has a raised seal on it from the State or County where they were born. Each applicants must provide a Social Security number and a government issued photo ID. If either applicant was previously married, spouse death certificate or divorce decree must be provided at time of application.
When does It Expire:
The Marriage License/Certificate is valid for 30 days from the date of issuance.
Name Change:
Filing for a marriage license in the state of Wisconsin does not cause your or your spouse’s last name to automatically change. Married name change services are available to guide you through the married name change process if you opt to change your name.
