A government issued photo ID, proof of age and Applicants who have divorced must show a certified copy of their divorce decree.if divorced within the last 6 months.
How To Apply:
Both applicants must appear in person together at one of the Cook County Clerk's six locations, must be 18 years of age or older, Fill out and sign a marriage license application, government issued photo id and proof of age. Applicants who have divorced must provide the date the divorce was finalized upon applying for a marriage license. Applicants who have divorced within the last six months must provide a certified copy of their divorce decree.
When does It Expire:
The license expires sixty (60) days after it becomes effective.
Name Change:
Filing for a marriage license in the state of Illinois does not cause your or your spouse’s last name to automatically change. Married name change services are available to guide you through the married name change process if you opt to change your name.
