What do I need to bring?
Proof of ages, photo ids and Social Security numbers. If previously married, his or her name, along with the date, location and court in which the marriage or civil union was dissolved or declared invalid, or the date and location of death of the former spouse or partner.
How To Apply:
Proof of age of each individual, at least 18 years of age at the time of the marriage. Acceptable documents for proof of age are a valid driver’s license or a certified copy of a birth certificate. If either individual was previously married or joined in a civil union, his or her name, along with the date, location and court in which the marriage or civil union was dissolved or declared invalid, or the date and location of death of the former spouse or partner.
When does It Expire:
The license expires 60 days after its effective date

Filing for a marriage license in the state of Illinois does not cause your or your spouse’s last name to automatically change. Married name change services are available to guide you through the married name change process if you opt to change your name.
