What do I need to bring?

A government issued picture ID, certified birth certificate, government document to prove permanent residency, and Social Security numbers. If either applicant was previously married, spouse death certificate or divorce decree or annulment.

How To Apply:

Both applicants must be present with a valid government photo identification, certified birth certificate and a Social Security number. If you have been married before, you will need to show a certified copy of the final divorce decree, with court raised seal, of your divorce or annulment decree or spouse death certificate.

When does It Expire:

The license is good for 30 days after the date of issuance and the marriage must be performed within the 30 day time-period.

Name Change:
Filing for a marriage license in the state of Wisconsin does not cause your or your spouse’s last name to automatically change. Married name change services are available to guide you through the married name change process if you opt to change your name.
