What do I need to bring?

Certified birth certificate, valid U.S. Government issued pictured ID card and valid Social Security CARD



How To Apply:

If one of you is a resident of the county you are applying for the certificate in, only one of you has to be present. If both of you reside outside of the county or state, both of you must apply in person. Both applicants must provide their Social Security numbers and verification of the number. Applicants may present a Social Security Card or a W-2 Form or company pay stub with name and number for verification.

After 60 days

Filing for a marriage license in the state of Ohio does not cause your or your spouse’s last name to automatically change. Married name change services are available to guide you through the married name change process if you opt to change your name.
