Birth certificates if under 25 years of age, valid drivers license/state ID with current address, application if chosen to print and fill out, a certified or true copy of divorce judgment, if divorced and social security numbers/cards
How To Apply:
Both must appear in person at the county clerk's office to apply. You can either print out the application online, fill it out and bring it in or fill out the application at the county clerk's office. If under 25 years of age, both must bring birth certificates. Both must also bring valid driver's license/state-issued I.D., showing correct address. Also if previously divorced, you must bring a certified or true copy of divorce judgment
