What do I need to bring?
Picture ID of applicant applying in person, and a government issued IDs and Social Security numbers are required of both applicants. If either applicant was previously married, divorce decrees, death certificate or court orders for ALL previous marriages is required and must include place and date of granted divorce or death of spouse.

When does It Expire:
The license must be used within 6 months of the effective date.
Name Change:
