Certified birth certificate, valid U.S. Government issued pictured ID card and valid Social Security CARD

Both parties must have an original birth certificate. If you are under the age of 18 you must have your original birth certificate and both parents must sign the marriage license. If one parent has custody, a certified copy of the custody papers must be presented. If a party is under the age of 16, in addition to parents written consent, you must have a court order from the judge.
