How To Apply:
Both applicants are required to provide a valid driver's license, state issued ID, or passport for identification. If previously married, and the last marriage ended by death, dissolution (divorce) or annulment, the Month, date and year of the previous marriage, County and State in which the death, dissolution (divorce) or annulment occurred. If the dissolution or annulment occurred within 30 days of the date you are obtaining the marriage license, you are required to provide a copy of the decree signed by the Judge of the Circuit Court.
When does It Expire:
The license is valid for 60 days after the effective date.

Filing for a marriage license in the state of Illinois does not cause your or your spouse’s last name to automatically change. Married name change services are available to guide you through the married name change process if you opt to change your name.
