How To Apply:
If you are under the age of 18: Both parties must personally appear at the Register of Deeds office. Both parties must present a certified birth certificate. Both parties must present a valid government picture ID card (driver's license, passport, etc.) that includes date of birth information. A parent or legal guardian must be present and present legal documentation to prove they are the parent or legal guardian. You may be required to produce a certified copy of court orders. A valid Social Security Number is required or an affidavit which indicates that you are not eligible for a Social Security Number.
